Pre-Qualifications:
- Insureds must have current plus 4 years prior work comp history
- Mod 1.20 or lower
- Illinois domiciled companies
- Some classes of business not accepted
If you would like to participate, the following information is required to submit:
- Acord Application or MTRMA Application
- Current Mod Worksheet
- Currently Valued loss runs for current plus 4 prior years
- Details on any loss over $25,000
