- Trended loss ratio of 65% or less
- Must be, or become, a member of the MADA
- Payment based on monthly reporting, with a 25% escrow deposit

- Current experience mod worksheet
- Audited premium for previous four years, plus estimated premium for the current year
- Audited payroll for previous four years, plus estimated payroll for the current year
- Current, plus four years of recently valued loss information (within six months)
- Details on any loss over $25,000
- Detailed narrative of operations
