INSTRUCTIONS

In order to take the quiz you will need to complete the Sign In information. Please consult your safety director if you are unsure what association, member, or test to select.

User Type - From the drop down menu of selections please select whether you are an Internal or External User
Association - From the drop down menu please select your Association. Move mouse (arrow) to the down arrow (symbol) and click to get the list of Associations. Click mouse (arrow) on your Association. This is required only if you are an External User and selected user type as External.
Member - From the drop down menu of selections please select your company’s name. Move mouse (arrow) to down arrow (symbol) of Member/Location and select appropriate location with mouse (arrow). This is required only if you are an External User and selected user type as External.
Test - From the drop down menu of selection please select the test you would like to take.
First Name - Type in your first name as you would like for it to appear on the course certificate.
Last Name - Type in your last name as you would like for it to appear on the course certificate.
User Id - Type in your Losscontrol user id. This is required only if you are an Internal User and selected user type as Internal.
Password - Type in your Losscontrol password. This is required only if you are an Internal User and selected user type as Internal.
