- $25,000 written manual premium
- Illinois exposure only
- NCCI mod of 1.25 or less
- Trended loss ratio of 70% or less
- In business at least five years
- Must be, or become, a member in good standing of one of the sponsoring associations
- Payment based on monthly reporting

- Current experience mod worksheet
- Audited payroll for previous four years, plus estimated payroll for the current year
